Enery Work Consent and Release Statement

I understand that the energy work session given involves a method of energy balancing for the purpose of holistic wellness. I understand very clearly that these treatments are not intended as a substitute for medical or psychological care.

I understand that energy work practitioners do not diagnose conditions, nor do they prescribe medicines, nor interfere with the treatment of a licensed medical professional. It is recommended that I seek a licensed health care professional for any physical or psychological ailment I have.

I agree to release, indemnify, defend, and hold harmless Lindsey Walker (the “Provider”), its owners, employees, agents, and contractors from and against any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of or relating to the service(s) provided by the Provider; my own acts or omissions; the Provider’s employees, agents, or contractors; and any third-party claims arising from or in connection with the service(s). This obligation to indemnify applies regardless of whether such claims are caused in part by the negligence of the Provider, except where prohibited by law.

I understand that this agreement shall be governed by and construed in accordance with the laws of the State of Texas. The parties agree that any disputes under this Agreement shall be resolved in the courts located within said jurisdiction.

I understand that this agreement constitutes the entire understanding between the parties regarding the subject matter hereof and supersedes all prior oral or written agreements.
